Provide a problem category from list above, plus the address that needs service.
For problems needing lots of detail use DC's on-line Citizen Service Request Form.

Ensure you record the tracking number, agency assigned, and target completion date.

DC Homeowners Center - The Homeowners Center will be open to the public between the hours of 8:30 am and 4:30 pm, Monday through Friday, to provide expert assistance to District homeowners seeking building permits for home improvement projects. Office of Community Relations and Services - The Office of Neighborhood Services and The Mayor's Office of Community Outreach have been placed together in the new Office of Community Relations and Services. The Acting Director is Merrit Drucker, and his Executive Assistant is Charlotte Hamilton, 202-727-3000. The Ward 3 Neighborhood Services Officer is Abby Frankson, 202-717-3624, abby.frankson@dc.gov.
